OBJECTIVE:
You may have decided that now is the time to transition from a medical model of care to a person centered model of care.  If so, this conference will help you clarify the principles and values of culture change, how to overcome potential barriers, and hear about lessons learned from those who have gone through the process.  NYSDOH will present a breakout workshop on “how to satisfy the state regulations while being more flexible with respect to resident choice and life style.  You will learn how to initiate and sustain the transition to person centered care.
